Breast Reduction and Lift

Breast Reduction and Lift

If you’re considering breast surgery, one of the most common questions patients ask is: Do I need a breast lift or a breast reduction, or, a breast reduction and lift?

Understanding the key differences between a breast lift (mastopexy) and a breast reduction (reduction mammaplasty) can help you make an informed decision and feel confident moving forward. An informed patient is an empowered patient.

Below, we’ll break down what each procedure does, who it’s best for, and how to know which option may be right for you.

What is a breast lift (mastopexy)?

A breast lift, or mastopexy, is a procedure designed to raise and reshape sagging breasts without removing a large amount of breast tissue.

Over time, factors such as pregnancy, breastfeeding, weight loss, aging, and gravity can cause the breasts to lose firmness and sit lower on the chest. A breast lift restores a more youthful contour by:

  • Elevating the breast tissue
  • Repositioning the nipple and areola
  • Tightening excess skin
  • Improving breast shape and projection

A Breast Lift Does Not Significantly Reduce Breast Size

One of the most important distinctions is that a lift primarily focuses on shape, not volume. Patients who like their breast size but want them higher and firmer are often excellent candidates for a lift. If you like the size of your breast in a bra, but don’t like how low the breasts are without a bra, a breast lift is all that may be needed.

What is a breast reduction?

A breast reduction is performed to reduce the size and weight of overly large breasts while also improving shape.

In addition to aesthetic benefits, breast reduction is often chosen for physical relief. Patients with large breasts may experience:

  • Neck, shoulder, or back pain
  • Bra strap grooving
  • Skin irritation beneath the breasts
  • Difficulty exercising
  • Challenges finding clothing that fits comfortably

During breast reduction surgery, excess breast tissue, fat, and skin are removed to create a smaller, lighter, more proportionate breast size.

Does a Breast Reduction Include a Lift?

Because tissue is removed and the breast is reshaped, breast reduction almost always results in a significant lift as well. The size of the areolae are often reduced too. Many patients are surprised to learn that surgery provides both functional and cosmetic improvement when they get a breast reduction and lift.

How Do You Know Which Surgery You Need?

A helpful way to think about it is:

  • If your main concern is sagging and you like the volume of your breasts in a bra → you may need a lift
  • If your main concern is heaviness or large size causing symptoms→ you may need a reduction

Do Breast Lift and Breast Reduction Use the Same Incisions?

In many cases, yes. Both procedures often use similar incision patterns, such as:

  • Lollipop incision (around the areola + vertical line)
  • Anchor incision (adds a crease incision for more extensive reshaping)

The best technique depends on your anatomy, goals, and degree of sagging or excess volume.

Recovery Differences

Recovery timelines are quite similar:

  • Most patients take 1–2 weeks off work
  • Light walking begins immediately
  • Resume wearing an underwire bra after 4 weeks
  • Exercise is usually resumed around 4–6 weeks
  • Final shape continues to settle over several months

Breast reduction and lift patients may experience slightly more initial soreness due to the larger amount of tissue removed, but both procedures follow a gradual healing process.
